Speech Language Pathologist
Home Health Setting

Provider of skilled nursing care and rehabilitation services to patients in their homes.

Responsibilities:
Provides diagnostic services to homebound patients as referred by a physician
Completes written reports of evaluation finding and provides recommendations
Develops and implements treatment plans
Collaborates with other home health providers in treating patient
Demonstrates and educates therapy for family member or responsible person who may assist with patient's rehabilitation
Re-evaluates patient's progress and therapy making appropriate changes as necessary
Provides in-service training and training programs to other home health staff
Provides appropriate referrals when needed

Requirements for this job:
Master's Degree in Communicative Disorders from an accredited university
Current or pending state license in practicing state
Able to qualify for a certificate of clinical competence (CCC) in Speech from the American Speech and Hearing Association
Current driver's license within the state of practice and minimal automobile insurance
Current CPR Certification required
