Physical Therapist
Outpatient Facility

The facility provides a full spectrum of health care services for patients and the community. We are committed to achieving optimal health by constant improvement of the facility and our delivery of health care services.

Responsibilities:
Responsible for providing physical therapy services to patients and/or customers through evaluation, treatment planning, implementation of care and education
Provides documentation, and communicates in a manner consistent with hospital policies, procedures, guidelines and protocols
Monitors aide training and uses Rehabilitation aides with treatment when appropriate
Works with multi-disciplinary team

Requirements for this job:
Bachelor's Degree in Physical Therapy from an accredited school of therapy
Current state license within the state of practice
One year experience required
Will be required to pass pre-employment drug test
CPR Certification required
