Job Description

Physical Therapist- Home Health
Full Time Days

Provides physical therapy assessment, treatment, and instruction to patients under the direction of a physician.
The therapist completes assessment, treatment, and other related paperwork in a timely fashion.
Collaborates with the case manager to provide home health services that reflect the home health care agency philosophy and standards.

REQUIREMENTS INCLUDE:
Current Oregon license in Physical Therapy required.
Current Healthcare Provider BLS required.
Current Oregon Drivers license and at least 4 years of holding a driver's license.
No more than 1 accident or moving violation on or off the job within the past 12 months and no conviction for driving under the influence of intoxicants (DUII) within the past 5 years.
Current automobile accident insurance with coverage limits of $100,000/$300,000/$100,000 or combined $300,000 limits and name the facility as the "additional insured".
One-year experience in home health or other health setting with geriatric emphasis preferred.
Provides care appropriate to the age of patients served on the job.
Knowledge of physical therapy assessment and treatment methods required.
