Physical Therapist
Will travel between two facilities
Sub Acute / Long Term Care Setting

Responsible for evaluating/assessing needs of referred patients and formulating treatment plans

May supervise Physical Therapy Assistants, aides

Requirements for this job:
Current license within the state of practice
Graduate of an accredited therapy program
Familiar with standard therapy concepts, practices, and procedures
