Requirements for this job:
Current or pending state license in practicing state
Graduate of an accredited therapy program

Responsibilities:
Performs assessments which correlates to the age of the patient and documents results; completes an initial written assessment of referred patients with established time frames; completes an initial plan of care based on assessment results within one working day.
