Provides physical therapy services to acute and chronically ill patients of all ages in the home
Provides care which supports the quality and productivity standards outlined in policies and procedures

Requirements for this job:
Graduate from an approved Physical Therapy program
Current license within the state of practice
Six months clinical experience
Current drivers license within the state of practice
Must have reliable transportation and proof of of acceptable motor vehicle liability insurance
CPR Certification
