This rehabilitation center specializes in the needs of older adults seeking to maintain maximum functional levels. We develop and manage programs that fulfill those needs appropriately, efficiently, and cost-effectively programs. We partner with senior care networks to bring the resources of highly professional and effective rehab and wellness programs to as many older adults as possible. Using the best practices and advanced clinical programming, we help facilities to achieve measurably improved patient outcomes while assuring successful rehab operations.

Responsibilities:
Review and evaluate physician's referral and patient's medical record to determine physical therapy
program required
Evaluate each patient's current level of functioning using approved evaluation techniques, tests and observations
and prepare written treatment programs based on evaluations
Administer a prescribed physical therapy program to restore and maintain maximum function
Coordinate physical therapy services with other therapy departments and the nursing department
Maintain appropriate documentation in the patient chart, and re-evaluate patient progress and
revise treatment programs when necessary

Requirements for this job:
Graduate from an accredited Therapy Program
Current state license in the state of practice
Acute care, long-term care and/or rehabilitation background
