PT ? Physical Therapist Liberal, KS
The Physical Therapist provides and directs Physical Therapy services to residents to include assessment and treatment, development and implementation of programs, and related professional in-services. Performs duties under the direction of physician?s orders.

Responsibilities:
? Provides Physical Therapy services to residents and initiates referrals when appropriate and in accordance with Physical Therapy Department procedures.
? Conduct screenings of residents at regular intervals to determine need for intervention/treatment.
? Evaluates residents to obtain data necessary for treatment planning and implementation. Conducts specialized evaluations as indicated.
? Interprets and communicates evaluation findings to residents, family, and other team members.
? Monitors resident?s response to intervention and modifies treatment as indicated to attain goals in accordance with physician?s orders.
? Participates in discharge planning including programming to maintain and enhance the performance of the resident in his or her own environment.
? Documents results of resident?s assessment, treatment, follow-up, and termination of services.
? Reviews the quality and appropriateness of the total services delivered and of individual Physical Therapy programs for effectiveness and efficiency, using predetermined criteria.
? Supervises Rehabilitation Aides, students, volunteers, directs LPTAs in the implementation of the treatment plan
? Assumes responsibility for ongoing continuing education and professional development
? Complies with all Infection Control, Universal Precautions and OSHA standards for the health care professional.
? Assists with and participates in Quality Assurance Programs as required.
