Job Description

Physical Therapy Assistant
Sub Acute / Long Term Care Facility
Will travel between Beachwood and Cleveland Facilities

Responsibilities:
Treats patients as directed by supervising physical therapist
Documents treatments given in the medical record according to established policy
Participates in patient care conferences and rehabilitation team meetings
Communicates with supervisor and other team members regarding patient progress, problems and plans
Participates in facility in-service training programs
Records daily treatment changes according to facility procedure
Assists with maintaining a clean safe treatment area and department
