Physical Therapy Assistant
Home Health Setting

Will cover as far as 50 miles from the home facility, however, we do not go 50 miles in every direction. We geographically assign patients so the staff is not going more than one direction on a given day. Approximately 6-7 visits per days. It is 8 hour shifts, roughly 8-5 although we are somewhat flexible on start/finish times (7-4 or 9-6). Home care experience would be helpful although not mandatory.

Responsibilities:
Provides treatment based on plan of care developed by the physical therapist
Completes and submits documentation in an accurate and timely manner
Administers physical therapy treatment to patients with a wide range of health problems, under the direct orders of a physician/podiatrist and under the direct supervision of a physical therapist
Explores patient's needs while implementing long and short-term objectives that are consistent with the organization's mission, values and goals

Requirements for this job:
Associate's degree from an accredited Physical Therapy Assistant Program
Current Physical Therapy Assistant license within the state of practice
BLS/CPR Certification
