OCCUPATIONAL THERAPIST
PACE?the first rural Program of All-Inclusive Care for the Elderly (PACE) on the Western Slope.
Our mission is centered around the belief that it is better for the well-being of seniors, with chronic care needs, and their families to be served in the community whenever possible. This is an innovative program that provides all needed preventive, primary, acute and long term health care services for eligible participants
Position Objective:
Under the supervision of the Center Director, provides occupational therapy services to participants of the Senior CommUnity Care. Provides clinical occupational therapy evaluation and treatment for program participants, on acute, restorative or maintenance levels as needed by Day Center, Nursing Home or in-home setting. Integrates the occupational therapy treatment plan into the Interdisciplinary Team plan of care. Assists in the ordering, inventory, distribution and maintenance of durable medical equipment for participants.
Responsibilities of a Occupational Therapist:
Conducts an in person initial comprehensive occupational therapy assessment on all Senior CommUnity Care participants upon enrollment. Coordinates with the Interdisciplinary Team to develop a comprehensive plan of care for each participant.
Functions as a member of the Interdisciplinary Team. Maintains regular attendance at, and participates in, Interdisciplinary Team meetings; communicates participant changes, collaborates on plan of care decisions and coordination for twenty-four (24) hour care delivery.
Provides treatment to participants and implements occupational therapy procedures consistent in achieving identified goals. Grades activities to facilitate goals. Confers with Interdisciplinary Team on participants care issues.
Performs competencies on appropriate direct care to staff as needed.
Assists in training and provides ongoing education and clinical supervision for Certified Occupational Therapist Assistant(s). Adheres to department policy and occupational therapy practice laws on Occupational Therapy Association practice and reports to direct supervisor regarding these issues.
Educates and instructs participants and family members.
Requirements
Requirements:
? Colorado registered Occupational Therapist
? Minimum of one-year experience working with a frail or elderly population
? Valid driver?s license, proof of insurance, means of transportation
? Good working knowledge of physical, psychosocial and behavioral needs of the elderly population.
? Effective listening, and written and oral communication skills.
? Problem solving skills, ability to multi-task and apply good judgment.
? Working knowledge of practice boundaries for PT, PTA, OT or COTA
? Basic computer skills
? Must have medical clearance for communicable diseases and up-to-date immunizations before having direct participant contact
