Job Description

Partnering with the District Manager, you'll support an environment of continuous learning that is focused on advancing the highest clinical standards in five core areas including balance, complex disease, dementia, pain, and continence management. You'll do all this while bringing in Master Clinicians to share their expertise. All in all, you will be a role model for therapy staff, presenting a professional and positive attitude. You will seek out all ends to get patients the rehabilitation and wellness services they need.

Essential Job Functions
• Meets the resident's goals and needs and provides quality care by assessing and interpreting evaluations and test results; determining treatment plans in consultation with physicians, and by prescription. Helps residents accomplish treatment plan and secures necessary supplies and adaptive equipment for residents to facilitate independence
• Administers therapy statements according to a treatment plan approved by the attending physician. Directs treatments given by aides and assistants. Complies with company protocol for patient rounding
• Evaluates effects of therapy treatments by observing, noting and evaluating resident's progress; providing medically necessary treatment plan and modifications
• Completes discharge planning by consulting with physicians, nurses, social workers and other health care workers; Participates in the Q & A process for rehab related issues.
• Manages the appropriate therapy minutes per RUGS category for patients/residents
• Assures continuation of therapeutic plan following discharge by designing resident specific maintenance programs; instructing residents, families and/or caregivers in follow-up maintenance programs; recommending and/or providing assistive equipment; recommending outpatient or home health follow-up programs
• Accuracy in completion of MDS items per workflow model
• Documents resident care services by charting in resident and department records according to accepted regulatory, corporate and professional guidelines. Records daily treatment charges per corporate procedures. Maintains resident confidence and protects nursing home operations by keeping information confidential
• Protects residents and employees by adhering to facility infection control policies and protocols. Maintains safe and clean working environment by complying with facility and department procedures, rules and regulations. Complies with company protocol for denial management. Ensure appropriate daily billing of services
• Ensures operation of equipment by completing preventive maintenance requirements; following manufacturer's instructions; troubleshooting malfunctions; calling for repairs
• Secures equipment, adaptive devices and supplies in accordance with company policy
• Maintains professional and technical knowledge by attending educational workshops; reviewing professional publications; and establishing personal networks
• Complies with federal, state and local legal and professional requirements by studying existing and new legislation; enforcing adherence to requirements; advising management on needed actions
• Travel required as needed to perform job
• Provides community based services such as health and wellness activities as required. Contributes to team effort by accomplishing related results as needed. Complies with company requirements on the completion of outcome measures. Willingness to provide services in multiple sites as determined by patient/resident need. Maintain specified efficiency level as determined by supervisor

• Assist rehab team in coordination of optimal interdisciplinary patient scheduling and establishing a group treatment schedule that is most appropriate to patients' needs
• Effectively communicate with MDS Coordinator and trans-disciplinary team to determine accuracy of appropriate minutes per rehab RUG category, and assures appropriate input
• Attend facility meetings as directed by DOR, to discuss rehab plan of care related to clinical management and appropriate use of minutes
• Communicate with DOR on all rehab related issues within facility in a timely manner. Relay communications from DOR to staff when requested
• Assist DOR in scheduling of staff around PTO absences and census fluctuations that assures proper staffing levels

Minimum Qualifications
• Graduate from an accredited program for Occupational Therapy
• Current license as a Physical Therapist, Physical Therapy Assistant, Occupational Therapist, Occupational Therapy Assistant, or Speech-Language Pathologist in the state of practice as required
• Must be able to travel as needed to perform job
• Must meet Clinical Competency requirements
• Capable of maintaining regular attendance

• Able to communicate in English, both verbally and in writing
• Possess problem-solving skills of the type and at a level necessary to accomplish the job
• Maintain confidentiality regarding patient and company proprietary information
• Ability to relate professionally, positively, and cooperatively with residents, resident's family members and other employees at all levels
• Must possess computer skills of the types and at a level necessary to accomplish the job
